In this location I'm getting 45 fps, but if I move forward a few feet or look in a different direction, it goes away. I'm uploading a video as I'm typing this, so I'll edit when it's finished. I also have an issue where my game gets 'dusty' a few feet in front of me, so it looks like things are loading in 10 feet ahead of me as I walk. I reinstalled the game, set it to Ultra, and haven't tweaked anything else. 
I don't think it is mods, as I've reinstalled my game and removed almost every mod. It doesn't seem to matter what settings I use, I've tried with everything tweaked optimally, and it still happens. My computer is certainly capable of handling this game with every graphics mod imaginable, but this happens anyway. The only thing that seems related is that it's almost always around NPC's.
